Field Trip Permission Form - This is the standard field trip permission form for the Roman Catholic Diocese of Albany schools.

Stewardship Approval Form - This is the form that needs to be completed for verification of stewarship.

Afterschool Application Form - This application needs to be completed for student participation in St. Mary's Afterschool Program.
